Underwriting Requirements
  • Eligibility: Full-time employees working at least 20 hours per week

Guaranteed issue
  • All coverage offered is guaranteed issue
  • No underwriting or submitting health questions required to qualify for coverage

Issue ages
  • Employee/contractor and spouse: 18-64
  • Dependents: 14 days – 26th birthday (varies by state)
